New patient forms are available online for you to complete prior to your visit. Many patients find it convenient to complete these documents at home where all of the information is available including insurance policies, medical histories, and contact information. If at any time you have questions or need assistance, feel free to call us. Forms are available when you arrive if you are more comfortable filling them out here.
We deal with dental insurance daily; therefore, we understand it better than most patients. We do not contract with any insurance company because of the limitations that it places on our doctor/patient relationship, however, we are more than happy to help you get the most out of your coverage. We will verify, estimate and file most PPO claims. If you have questions about your specific benefit plan, please ask one of our team members and we will do our best to find the answer.
For the out-of-pocket costs of treatment, we accept payment in full at the time of your visit. We take payment in the form of cash, check, or major credit card. We also work frequently with CareCredit, and Wells Fargo, both offer low and no interest financing plans for qualified patients for any treatment, even cosmetic dentistry and other services not typically covered by insurance. You can pre-qualify online, or one of our team members will be happy to help you complete your application in our office.
Patients sometimes put off dental treatment due to the cost involved. Often times, it isn’t because the patient lacks the financial ability to pay; but, they just want a convenient way to pay that works into their lifestyle. CareCredit allows you to pay over time. CareCredit is exclusive for healthcare services and you can get No Interest if paid in full within the promotional period. If you need more time to pay for your procedure, you can take advantage of their Extended Payment Plans with low fixed interest rates. We have many patients that enjoy the benefit of CareCredit.
